The VAD/Heart Transplant Coordinator will be responsible for the coordination of care for all patients requiring implantation of a cardiac assist device. This includes patients accepted for, and awaiting heart transplant, or those with advanced heart failure in which the device is implanted as a permanent treatment option. The coordinator will handle coordination for all patients pre‑ or post‑heart transplant. Working as a member of the Advanced Heart Failure Team, the coordinator will be the primary liaison with transplant cardiologists, cardiac transplant surgeons, other physicians, and healthcare staff throughout the hospital.
Responsibilities include patient and family education, daily rounds, discharge planning, on‑call coverage, outreach activities to promote the program, frequent communication with referring physicians, and professional education. The coordinator operates within the parameters outlined in the LVAD practice guidelines and adheres to ANA Nursing standards.
Minimum of 3 years acute care experience. Experience in acute care, cardiology, heart failure, or VAD care is preferred.
EducationAssociate’s degree in Nursing with a minimum of 10 years of relevant experience is required. Bachelor’s degree in Nursing with a minimum of 3 years of relevant experience is required. Bachelor’s degree in Nursing is preferred.
Special SkillsAnalytical skills to prepare and implement effective nursing care; ability to concentrate, pay close attention to detail for up to eighty percent of work time while planning and performing professional nursing care, resolving patient care problems, and interacting with patients, families, and other professionals, often during stressful situations.
LicensureCurrent Wisconsin Registered Nurse License or a multi‑state nursing license from the NLC. Preferred certifications:
Certified Clinical Transplant Coordinator (CCTC), Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN).
