1. Shift Details: 7/70 schedule, 10 hour shifts. This role will focus on hospital-based inpatient unit work.

Job Summary

The RN Case Manager coordinates the provision of care and services of patients on the Inpatient Rehabilitation Unit through their stay and across the continuum. The RN Case Manager works collaboratively with interdisciplinary teams and team members both internal and external to the organization to improve patient care by assuming a leadership role in driving desired clinical and financial outcomes through effective utilization and monitoring of healthcare resources.

The RN Case Manager utilizes standards, guidelines, and protocols for care delivery and incorporates data to continuously improve care and outcomes.

Responsibilities
  • Coordinate the provision of care and services for patients on the Inpatient Rehabilitation Unit throughout their stay and across the continuum.

  • Utilize standards, guidelines, and protocols for care delivery and incorporate data to improve care and outcomes.
Experience

A minimum of 3 years of clinical nursing is required. Five years of clinical nursing and case management experience is preferred. Experience in case management including transitional planning, outcomes management and assessment is preferred.

Education

Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required. A Bachelor's degree is preferred.

Training

Accredited case manager certification (ACM-RN) through the American Case Management Association (ACMA) is preferred.

Special Skills

General knowledge of payor industry (Medicare, Medicaid, & Commercial insurance regulations), resource management, and evidenced based clinical practice is essential. Theoretical knowledge of the nursing and case management processes. Analytical skills necessary to assess patient s discharge needs and develop solutions to problems concerning patients  mental status or physical well-being to ensure optimal patient outcomes. Advanced communication and interpersonal skills with all levels of internal and external customers including medical staff, patients and families, clinical personnel, support and technical staff, outside agencies, and community members.

Other skills include excellent written/verbal communication, critical thinking, creative problem solving, proficient organization and planning. Basic computer skills. Must be self-directed with the ability to tolerate frequent interruptions and a demanding workload.

Licensure

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the Nurse Licensure Compact (NLC).
