Job Summary

The Interventional Radiology MSK RN Coordinator is responsible for the coordination of care of patients within the Musculoskeletal (MSK) current and future programs. Current programs include the Vertebral Compression Fracture Pathway and Skeletal Metastasis Program. The coordinator is responsible for assessing and triaging new patient referrals, coordinating patient clinic visits and follow up, reviewing clinic processes, developing and implementing clinical workflows, working collaboratively with interprofessional multidisciplinary teams to support MSK IR research projects, developing and implementing patient and staff education resources, and developing quality initiatives to promote the safe and efficient care of the MSK patient population served within the IR clinic.

The Clinical Coordinator - Ambulatory Clinics supports a specialty population and/or group of specialty patients by coordinating patient care at the local level, across sites and settings, and/for the duration of the care continuum. The Clinical Coordinator facilitates the development and provisioning of clinical resources; assists with evidence-based practice & quality improvement initiatives and facilitating specialty training and orientation. Required skills include critical thinking, excellent verbal and written communication, the ability to work autonomously/be self-directed, collaborating with others, acting as a change agent, and ability to teach peers.

The Clinical Coordinator is able to complete all duties of the Staff RN role in their designated care area.

In addition, this nurse works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. This role systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The nurse utilizes evidence to drive clinical decisions and practice related to individual and population health.

The nurse establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The nurse considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The nurse delegates and supervises care in accordance with the WI Nurse Practice. This role participates in continuous quality improvement initiatives and assumes responsibility for their professional development.

Experience

A minimum of 3 years as a nurse in a healthcare setting required. Candidates with two years as a nurse in the listed specialty (i.e. dermatology, cardiology, etc.) may be substituted for three total years of nursing experience. Experience in the specialty strongly preferred.

Education & Licensure

Nursing Licensure Required. Bachelor’s Degree in Nursing, Preferred.

Licensure:
Requires current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days of hire.
