RN Hospice - Weekend Program

Home Hospice / Field RN (No case load)
This RN will work exclusively in the Weekend After-Hours Hospice Program, supporting patients and families with end-of-life needs across the Milwaukee North region. Unlike traditional Hospice Case Managers, this role does not carry a caseload. Instead, the RN responds to daily scheduled visits, PRN symptom management, and pronouncements/death visits as needed.
The nurse functions independently in the field, providing compassionate, timely hospice care while collaborating with on-call teams.
Key Responsibilities- Complete daily scheduled hospice visits, focusing on comfort care and symptom management.
- Provide PRN support for urgent needs, including pain crises, rapid decline, or unexpected changes in condition.
- Conduct pronouncements/death visits, follow required documentation procedures, and support families during immediate bereavement moments.
- Assess and manage symptoms such as pain, agitation, respiratory distress, nausea, and anxiety.
- Communicate patient changes to the interdisciplinary hospice team (MD, NP, RN Case Managers).
- Provide education to patients and families on medication, disease progression, and what to expect during the end-of-life process.
- Document care accurately and timely using the facility’s EMR system.
- Travel across assigned service areas within required time frames using personal transportation.
- Follow all regulatory and organizational protocols regarding hospice care, safety, and compliance.
- Active RN license (Wisconsin or compact).
- Minimum 1 year of recent hospice experience preferred.
- Strong skills in symptom management and end-of-life care.
- Valid BLS certification.
- Must be comfortable working independently in the field.
- Reliable transportation required.
- Ability to work all assigned weekend and holiday shifts (if holiday falls on a weekend).
