Position: Case Manager - Full-Time/Part-Time/Casual All Shifts
Overview
RN Case Manager - Hospice — Full-time hours. Mileage reimbursement provided. Wages up to $45/hr (based on experience). Looking for RNs in the Milwaukee or Wausau area with some hospice experience. Summary/Objective
Responsible for planning, organizing, developing, and directing the nursing care of the hospice patient in accordance with current federal, state, and local standards, guidelines, and regulations.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Assisting the admission nurse in the coordination of timely admissions.Provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care.Assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life.Assist in facilitating freedom of choice, relief of suffering, and death with dignity for patients facing end-of-life issues.Educate patient and family on the disease process and expectations related to disease progression.Provide support and education for families based on the hospice philosophy of care, including physical, psychological, spiritual, and financial comfort.Participate in Interdisciplinary Team meetings and update the Plan of Care (POC) for assigned patients based on identified problems and needs.Ensure patient care is delivered at the appropriate level of care.Coordinate with PCP and medical director to manage medication profiles to prevent polypharmacy and ensure optimal symptom management.Manage medication profile to ensure hospice provides medications related to disease process and symptom management.Ensure all services provided are in accordance with the patient’s Plan of Care and actively involve the patient, family, Primary Care Physician, and the Interdisciplinary Team.Provide team members with appropriate information to coordinate all services related to hospice care for assigned patients, including assigning visiting nurses, home health aides, chaplains, social workers, volunteers, and ancillary services; order medications, Durable Medical Equipment, and supplies as needed.Provide consultation and supervision to CNAs and LPNs involved in the care of the patient.Participate in performance evaluations of clinical members of LPNs and CNAs involved with the caseload.Assist with orientation of clinical members participating in the care of the assigned caseload.Ensure cost-effective practices by self and others; assist with QAPI (Quality Assurance and Performance Improvement) activities.Work as a team member at all times and uphold the Hospice Code of Ethics.Provide care to manage patient needs and utilize the medication formulary whenever possible for the management of patient symptoms.Triage scheduling and symptom management calls from members of the Interdisciplinary Team.Report to the Clinical Director as patient condition changes or as changes in the level of care are required.Work Environment/Physical Requirements
The responsibilities involve significant physical activity including standing, lifting, bending, stooping, pushing, pulling, and twisting. The employee must be a licensed driver with a vehicle in good working order and insured as required. Travel and weather conditions may affect work; must be able to adapt to meet patient care needs.
