MILWAUKEE at our FROEDTERT HOSPITAL facility.

Job Type: Full-time FTE: 1 Shift: Shift 1:
Monday - Friday 8:00 am - 4:30 pm Hybrid: 2-3 days in clinic, remaining remote.

Job Summary Provides primary clinical care management and coordination of care for assigned patients under the direction and clinical supervision of the medical director or designee for the program. Services may include direct patient care according to scope of practice, coordination of program resources, coordination of quality improvement initiatives, and coordination of the continuum of care to these patient populations as well as to any patient in the General Surgery Clinic.

Other duties as assigned. Experience

Three to five years of experience in the nursing field and hands‑on patient experience in an acute care setting is required. General Surgery or Med‑Surg experience is preferred.

Education

Professional knowledge of nursing theory and practice at a level normally acquired through the completion of a Bachelor’s degree in Nursing from an accredited School of Nursing is required. Bachelor’s degree is preferred.

Special Skills

Knowledgeable in the care of patients undergoing general surgery procedures.

Licensure

Requires current state of Wisconsin Registered Nurse License or a Multi‑state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 3 months of hire. Medicaid and Medicare (PECOS) enrollment is required.
