The Staff RN-Ambulatory Clinics works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. The Staff RN-Ambulatory Clinics systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The Staff RN-Ambulatory Clinics utilizes evidence to drive clinical decisions and practice related to individual and population health.
The Staff RN-Ambulatory Clinics establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The Staff RN-Ambulatory Clinics considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The Staff RN-Ambulatory Clinics delegates and supervises care in accordance with the WI Nurse Practice. The Staff RN-Ambulatory Clinics participates in continuous quality improvement initiatives and assumes responsibility for their professional development.
Major accountabilities include patient care coordination, direct patient care, patient education, telephone triage/eVisits, and performing/assisting with in-clinic minor procedures. May float to other areas as needed for operations needs. Other duties as assigned.
- EXPERIENCE
DESCRIPTION:
Clinical experience and/or interest in the care and management of patient populations specific to the area of employment. At least 1 year of relevant nursing experience, preferred. - EDUCATION
DESCRIPTION:
Nursing Licensure Required. Bachelors Degree In Nursing, Preferred. - LICENSURE
DESCRIPTION:
Requires current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC. American Heart Association BLS required within 90 days of hire.
