Responsibilities- Serve as a liaison between the patient and family, the Agency, and member hospital; develop ongoing relationships with referral contacts, respond to referrals in a timely manner and develop a plan to support growth.
- Communicate and collaborate with the Hospice Team regarding admission information and any changes to the patient status that could impact the patient admission process.
- Complete admissions: conduct a thorough patient assessment that allows for the development of a transition plan and plan of care that will ensure comfort, safety, and peace of mind.
- Interact and collaborate with patient, patient family and/or significant caregivers to coordinate referral to services.
- Ensure goals are clearly delineated and prioritized on the patient plan of care and addressed by all team members.
- Ensure plan of care reflects assessment findings to establish appropriate goals and interventions to meet patient needs and that goals are realistic and measurable.
- Conduct case conferences with team members regularly which includes creating PCW/HHA care plans, initiating and updating plans in accordance with patient needs and agency policy.
- Inform physician, clinical manager and interdisciplinary team members of changes in patient status as evidenced by case communication documentation.
- Assure appropriate discharge planning occurs from the start of care by utilizing a multidisciplinary approach and appropriate documentation.
- Degree from an Accredited School of Nursing (BSN preferred)
- Current WI RN license
- Related nursing experience preferred
- Current/Valid WI Driver’s License with reliable, insured vehicle

Full-time, Monday‑Friday 8:00 a.m.‑4:30 p.m.
Salary, Exempt Hospice Virtual Inpatient Unit.
