RN CASE MANAGER – Froedtert Health
Location:
Milwaukee, Wisconsin – Froedtert Hospital, Inpatient Rehabilitation Unit.
On‑site position. Full‑time (FTE 0.875). Standard hours 35.00, Shift 1 (7am–5:30pm), 7 days on / 7 days off.
The RN Case Manager coordinates the provision of care and services of patients on the Inpatient Rehabilitation Unit through their stay and across the continuum. They work collaboratively with interdisciplinary teams and team members both internal and external to the organization to improve patient care by assuming a leadership role in driving desired clinical and financial outcomes through effective utilization and monitoring of healthcare resources.
The RN Case Manager utilizes standards, guidelines and protocols for care delivery and incorporates data to continuously improve care and outcomes.
Experience
: A minimum of 3 years of clinical nursing is required. 5 years of clinical nursing and case management experience is preferred. Experience in case management, including transitional planning, outcomes management and assessment, is preferred.
Education
:
Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years of education at an accredited School of Nursing is required for RN licensure. A bachelor’s degree is preferred.
Accredited case manager certification (ACM‑RN) through the American Case Management Association (ACMA) preferred.
Special SkillsGeneral knowledge of payor industry (Medicare, Medicaid and commercial insurance regulations), resource management and evidence‑based clinical practice is essential. Theoretical knowledge of nursing and case management processes. Analytical skills to assess patient discharge needs and develop solutions for mental status or physical well‑being. Advanced communication and interpersonal skills with all levels of internal and external customers including staff, patients, families, outside agencies and community members.
Excellent written and verbal communication, critical thinking, creative problem solving, and proficient organization and planning skills. Basic computer skills. Must be self‑directed and tolerant of frequent interruptions and a demanding workload.
Requires current Wisconsin Registered Nurse License or a multi‑state nursing license from a participating state in the NLC (Nurse Licensure Compact).
