Position: Nurse Practitioner (Family Medicine)
Join our team committed to the delivery of the highest quality healthcare service. We are seeking a full-time Nurse Practitioner who is self‑motivated, energetic, and a take‑charge individual.
Job Responsibilities Provides primary health care services to individuals and their families in accordance with approved privileges.Practices in collaboration with physicians, as defined by law.Provides care that emphasizes health promotion, wellness, and disease prevention.Diagnoses and manages acute and chronic health conditions in partnership with the patient.Participates in coverage of same day services including but not limited to urgent care, school‑based care, and telehealth. This will include daytime coverage and a minimum of three shifts of evening coverage per month depending on the staffing needs of the clinic.Works in partnership with a physician and one or more other advanced practice providers to care for a shared panel of patients on the medical team, which includes but is not limited to diagnosing and managing acute and chronic conditions.Includes patient in medical decision making.Orders, conducts, and interprets diagnostic studies and laboratory tests.Prescribes medication.Prepares and maintains medical patient reports and records.Participates in all required continuing education to maintain clinical skills and meet all licensure requirements.Attends all required SSCHC meetings including department meetings.Abides by all SSCHC policies and procedures.Performs other duties as assigned.Qualifications
Licensed as a Registered Nurse in the State of Wisconsin.Completion of a baccalaureate nursing program.Completion of a master's degree from an accredited Family Nurse Practitioner Program.Certified as a Nurse Practitioner.Licensed for prescriptive authority in the State of Wisconsin.Working knowledge of the social and cultural factors associated with the community's use of social and health care services.Ability to understand patient attitudes and needs, develop and orient program goals and concepts accordingly, and to work effectively toward their accomplishment with all staff members.Ability to establish and maintain effective working relationships with coworkers, officials, the public, patients, other private and public organizational representatives, health care personnel and community groups and organizations.Ability to work under pressure, maintain personal composure, evaluate situations, and act/react appropriately, and use tact in dealing with visitors, patients, and difficult situations.Ability to identify and maintain confidential material.Ability to interpret and conform program operations to applicable laws, rules, regulations, and funding source requirements and according to acceptable standards.BLS certification maintained.Spanish speaking preferred.#J-18808-Ljbffr
