This job description indicates the general nature and level of work expected of the incumbent; it is not comprehensive.

• Assess, diagnose, and determine or alter treatment and management plans appropriate for age, acuity, and clinical condition.
• Order, perform, and interpret appropriate diagnostic studies and prescribe pharmacologic and non‑pharmacologic interventions and therapies.
• Manage conditions based on clinical indication, evidence‑based care, cost effectiveness, and risk/benefit assessment.
• Provide health promotion, disease prevention, and counseling for patients and families.
• Manage patients as part of an interdisciplinary team and seek physician or other healthcare team member consultation or referral when appropriate.
• Escalate needs for emergent or specialized care when necessary.
• Maintain accurate, complete, concise, and timely documentation in the electronic medical record and ensure compliant coding and billing practices.
• Facilitate consistent, coordinated care and clear communication among all members of the healthcare team and health or community agencies.
• Perform office or hospital procedures in accordance with specialty practice, competency, and granted privileges (where applicable).

• Participate in quality, safety, and peer review initiatives and performance activities, departmental meetings, and committees.
• Participate in education and onboarding of new team members, students, and other health care professionals.
• Seek experiences to maintain and develop clinical and professional skills and advance the profession within and outside the organization.
• Maintain standards of productivity, access, face‑to‑face time, and quality metrics to ensure optimal, safe, and timely patient care delivery.
• Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.

• Active RN, APRN/APRN‑FPA/APNP, or other APRN license(s) in states of practice.
• Active national board certification in area of clinical practice and populations served.
• Active DEA registration prior to hire.
• Active Illinois Controlled Substance License prior to hire (if Illinois practice).
• Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire.

• Master’s Degree in Nursing.
• No experience required.

• High level of clinical proficiency and excellent decision‑making skills.
• Ability to work independently and as an effective team member.
• Adaptability to evolving technology and proficiency with electronic medical records.
• Excellent communication skills and ability to collaborate and build relationships.
• Proven organizational skills and ability to prioritize effectively.

• Stand, walk, bend, stoop, and twist continuously throughout the workday.
• Functional speech, vision, touch, and hearing.

• Lift up to 50 lbs from floor to waist.
• Lift up to 20 lbs over the head.
• Carry up to 40 lbs a reasonable distance.
• Operate all equipment necessary to perform the job.
