• Location: Milwaukee, WI 53214 – Work in a vibrant Wisconsin city known for its beautiful lakefront, diverse neighborhoods, and thriving arts and culinary scenes.
• Healthcare Setting: Home Health
• Start Date: November 10, 2025
• Contract Duration: 13 weeks
• RN Shifts: Friday, Saturday, Sunday (Blocked Schedule)
• Shift Hours: 12-hour shifts, 3 shifts per week (36 hours/week)
• Estimated Weekly Pay: $2,003 - $2,226

• Minimum 12 months of recent Home Health RN experience (within the last 3 years)
• Proficiency with HCHB (Homecare Homebase) – required
• OASIS documentation experience – required
• Active Wisconsin or Compact Registered Nurse License – required (must be in hand at time of submission)
• Current BLS certification – required

• Deliver skilled nursing care to patients in their homes across the Greater Milwaukee area
• Complete three patient admissions per day and approximately two additional home visits daily
• Utilize HCHB and OASIS for accurate, timely clinical documentation
• Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans
• Educate patients and families on disease management, medications, and self-care techniques
• Ensure compliance with all regulatory, clinical, and safety standards
