As a Clinical Liaison/Senior Advisor for Care Patrol of Milwaukee, you will work full-time in a remote capacity based. Key responsibilities include collaborating with families to identify appropriate care options, conducting assessments, coordinating with healthcare providers, and assisting with transitions into senior living communities. This role will also involve educating families, fostering relationships with care providers, and representing our services to potential clients and partners.
Qualifications- Experience in Case Management, Home Care, and Admissions to effectively assess and address client needs
- Sales and Business Development skills to build and maintain relationships with care providers and potential referral sources
- Strong interpersonal and communication skills to effectively guide families through the decision-making process
- Ability to travel locally within the service area and work on-site
- Familiarity with senior living options and local healthcare resources is highly preferred
- Bachelor's degree in a related field such as Clinical Liaison, Admissions, Social Work, or Senior Living is a plus
