Hospice Social Worker (MSW/BSW) – Home Hospice

Location:

Milwaukee, WI 53213
Job Type: Contract

Hours:

40 hrs/week

Shift: Day Shift (Mon–Fri)
Contract Dates:
March 2, 2026 – May 30, 2026

Job Summary

Facility is seeking an experienced Hospice Social Worker (MSW/BSW) to support terminally ill patients and their families in the Milwaukee area. This role focuses on psychosocial assessments, emotional support, and end-of-life planning in a home hospice setting.

Candidates must be independent, compassionate, and experienced in hospice or end-of-life care.

Unit & Caseload Information

  • Average Caseload: 35–40 patients per social worker

  • Daily Visits: 4–5 per day

  • Patient Population:
    Terminally ill patients (prognosis ≤ 6 months)

  • MSW Ratio: 35–40:1

Schedule & Shifts

  • Schedule:

    Monday – Friday

  • Hours:

    8:00 AM – 4:30 PM

  • Weekends:
    None

  • On Call:
    None

  • Holidays:
    None

  • Floating:
    None

Minimum Requirements

  • Master of Social Work (MSW) or Bachelor of Social Work (BSW)

  • Hospice

    Experience:

    Strongly Preferred

  • Travel

    Experience:

    Required (No first-time travelers)

  • Ability to Work Independently

  • Valid Driver’s License (Preferred for Home Visits)

Key Responsibilities

  • Complete initial psychosocial assessments and ongoing visits

  • Provide emotional and grief support to patients and families

  • Assist families with:

    • Advance Directives

    • Power of Attorney

    • DNR Discussions

  • Connect patients to community and financial resources

  • Support interdisciplinary hospice care planning

  • Document care plans and visits accurately

  • Participate in care conferences as needed
