Job SummaryThis position is responsible for developing and monitoring goal achievement and other documents as needed for all clients served with regular contact with the client’s care team.
Essential Functions- Provides assessment, program planning, and service coordination for clients, including maintaining deficiency‑free charts.
- Develop Individual Support Plans for all clients on caseload, writing SMART goals and reporting on goal achievement every six months.
- Develop Behavior Support Plans and Fall Prevention Plans as needed.
- Communicate internally with clients and coworkers and externally with guardians, community case managers, caregivers, transportation drivers, and others.
- Accurately account for clients’ money used for community outings.
- Manage and administer medication as needed.
- Occasional travel (0‑12%).
- All other duties as assigned.
- Bachelor’s Degree or Associate’s Degree in a related field with a minimum of one year of experience as a proven Program Specialist.
- Previous related experience in a healthcare setting is preferred.
- Able to work independently on a broad variety of projects.
- Exercises effective judgment, sensitivity, and creativity to changing needs and situations.
- Meets internal and external deadlines and produces a consistently high‑quality product.
- Can multitask, perform multiple priorities, make decisions, and solve problems.
- Contributes in a team environment and/or independently to provide excellent customer service.
- Organizes, plans, and executes tasks efficiently and professionally.
- Solves problems and thinks critically, even under stress.
- Strong written and verbal communication skills.
- Strong computer skills, especially with Microsoft Word, Excel, and Outlook.
- Has a valid Wisconsin Driver’s License and a clean driving record.
- Valid Wisconsin Driver’s License
- CPR/First Aid certified within first six months of hire
