Transition in Place (TIP) Case Manager

If so, we invite you to join our team as a Transition in Place (TIP) Case Manager, where your work directly supports homeless veterans on their journey to stability, independence, and permanent housing.
About the RoleAs a TIP Case Manager, you’ll be at the heart of our transitional housing program providing client-centered, strengths-based case management to veterans who have bravely served our country and now need support navigating life's challenges. You’ll work closely with veterans to remove barriers, access vital resources, and build the skills needed for long-term success. This role is ideal for someone who values collaboration, believes in harm reduction and motivational interviewing, and is energized by helping people achieve meaningful change.
WhatYou’ll Do Intake & Assessment
- Welcome and engage new residents through comprehensive intake assessments
- Identify eligibility, strengths, and immediate service needs
- Assess psychosocial, emotional, medical, and behavioral health factors
- Partner with residents to create and maintain Individualized Service Plans (ISPs)
- Set measurable, goal-oriented plans focused on housing stability, employment, health, and wellness
- Regularly review progress with residents and the interdisciplinary team
- Connect residents to internal and community-based services, including mental health care, substance use treatment, employment support, education, and housing resources
- Coordinate appointments, transportation, and follow-up services to ensure continuity of care
- Maintain daily contact with residents on your caseload
- Track goals, milestones, and outcomes
- Keep accurate, timely case notes and documentation in HMIS and other systems
- Respond calmly and effectively to resident crises
- Utilize de-escalation techniques and connect residents with emergency services when needed
- Advocate for resident rights and help navigate healthcare, legal, and social service systems
- Provide emotional support, guidance, and mentorship
- Educate residents on life skills, coping strategies, and independent living
- Facilitate or coordinate workshops and group sessions (budgeting, job readiness, wellness, etc.)
- Actively participate in team meetings, case conferences, and interdisciplinary discussions
- Communicate effectively with clinical staff, VA partners, community providers, and other stakeholders
- Ensure all work aligns with organizational policies and regulations
