What You'll Do:Key Responsibilities
Under the direct supervision of an RN Case Manager, you will provide hands‑on personal care and support based on a customized patient care plan. Your duties will include:
Provide Personal Care: Assist clients with bathing, personal hygiene, grooming, dressing, and other activities of daily living (ADLs).
Assist with Mobility: Safely transfer, position, and ambulate clients using proper body mechanics and equipment such as Hoyer lifts, slide boards, and sit‑to‑stand aids.
Execute Therapeutic
Activities:
Assist with prescribed range‑of‑motion exercises, application of non‑sterile dressings, and use of assistive devices.Perform Specialized Care: As directed by the RN, provide catheter care (including bag changes), colostomy care, and assistance with established bowel programs.
Support Nutrition & Household Needs: Plan and prepare nutritious meals according to dietary plans, assist with feeding, perform light housekeeping, and shop for groceries to maintain a clean and healthy home environment.
Collaborate and Document: Accurately document all care provided, report any changes in the client's condition immediately to the RN Case Manager, and maintain clear communication with the care team.
Maintain Professionalism & Safety: Adhere to all agency policies, including the Exposure Control Plan, and maintain a professional, supportive relationship with all clients, including those with complex medical or psychological needs.
Qualifications & Skills
Required:
Current Certified Direct Care Professional (CDCP) certification OR a minimum of two (2) years of professional direct care experience.
A valid driver's license, reliable insured vehicle, and willingness to travel to client homes.
Ability to perform the physical duties of the role, including lifting, transferring, and repositioning clients (with or without reasonable accommodation).
Must pass a pre‑employment physical and drug screening.
Demonstrated reliability, punctuality, and a strong sense of responsibility.
A positive, team‑oriented attitude with the ability to follow care plans and take direction from RN Case Managers.
Preferred:
Previous experience in a home health care setting.
Experience caring for clients with complex conditions such as dementia, ALS (Amyotrophic Lateral Sclerosis), MS (Multiple Sclerosis), or quadriplegia.
Proficiency in advanced care skills, including Hoyer lift transfers, catheter care, bowel programs, and range of motion exercises.
