Job Summary
This Clinical Ambulatory Pharmacist will support both Endocrine and Primary Care/Population Health. The pharmacist will have dedicated days each week split between the Endocrine and Primary Care/Population Health clinics. This pharmacist will be part of a clinical pharmacy team in the FMLH Endocrine clinic that provides chronic disease state management for type 1 and type 2 diabetes; osteoporosis and osteopenia; as well as weight loss.
Within Primary Care, they will help support population health initiatives and 340B expanded eligibility work. This pharmacist will serve as a medication resource, will help with the optimization of patients’ medication regimens, and may have other responsibilities, as assigned. In this role, the pharmacist will provide direct patient care via in‑person office visits, virtual visits, and telephone encounters. This is a hybrid position and will require the pharmacist to travel between clinics.
Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department’s quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.
ExperienceA minimum of 1 year hospital or 3 years of other pharmacy experience is preferred.
EducationBachelor’s degree in Pharmacy or Pharm.
D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred.
Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred.
LicensureActive Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months.
