Full Time Hours Available: 8:00AM-4:30pm, Monday-Friday

• Be the first point of contact for patients and families during one of life's most important moments.
• Play a pivotal role in coordinating seamless, compassionate care for managed care patients.
• Work closely with interdisciplinary teams, physicians, and referral sources to ensure every detail is perfect.

• Master the details:
  Accurately enter and manage patient data, ensuring every referral, admission document, physician order, and CTI is complete, signed, dated, and uploaded flawlessly into the HCHB system.
• Stay ahead of the curve:
  Monitor DME orders, medical supplies, physician signatures, and level-of-care billing accuracy with eagle-eyed precision.
• Be the communication hub:
  Handle incoming calls with warmth and efficiency, route messages accurately, and keep the entire team connected.
• Support the big picture:
  Prepare IDT agendas, update plans of care, mail/fax UPOCs, and create organized facility binders-because every detail matters.
