To work as an Advanced Nurse Practitioner within Oakridge Park Medical Centre, delivering high-quality care to patients presenting with minor illness, and supporting the management of long-term conditions (LTCs) and QOFtargets.

The post holder will work independently within their scope of practice while contributing to the wider multidisciplinary team.

Main duties of the job

Clinical Responsibilities

  • Independently assess, diagnose, and manage patients presenting with minor illness
  • Undertake comprehensive clinical assessments including history taking, physical examination, diagnosis, treatment, and referral as appropriate
  • Prescribe safely and effectively in line with current legislation and local guidelines
  • Request, interpret, and act upon investigations
  • Refer patients to secondary care or other services when clinically indicated
  • Maintain accurate and contemporaneous clinical records

Long-Term Conditions & QOF

  • Support the management and review of patients with long-term conditions e.g. asthma, hypertension
  • Contribute to achieving QOF targets through high-quality, evidence-based care
  • Proactive approach to patient care alerts
  • Work collaboratively with the practice team to improve patient outcomes

Professional & Team Working

  • Work closely with GPs, nurses, HCAs, pharmacists, and the wider MDT
  • Participate in clinical meetings, audits, and service development
  • Maintain professional registration and engage in ongoing CPD
  • Adhere to clinical governance, safeguarding, and control policies
About us

The Practice provides GP services to the expanding areas of Northern Milton Keynes. There are Three partners, and a large multi-disciplinary team of other professionals and support staff. All are committed to providing patients with a high quality and personal service.

The surgery

The building has been designed to provide a pleasant and welcoming environment and it is equipped to very high standards. There is a large free car park. Please park considerately to allow maximum use.

The area nearest the surgery entrance is reserved for disabled patients. For patients who are disabled there is easy access, and all consulting rooms are on the ground floor.

  • Contribute to achieving QOF targets through high-quality, evidence-based care
  • Participate in chronic disease clinics as required
  • Work collaboratively with the practice team to improve patient outcomes

Professional & Team Working

  • Work closely with GPs, nurses, HCAs, pharmacists, and the wider MDT
  • Participate in clinical meetings, audits, and service development
  • Maintain professional registration and engage in ongoing CPD
  • Adhere to clinical governance, safeguarding, and infection control policies
Qualifications
  • Registered Nurse (NMC)
  • Qualified Advanced Nurse Practitioner (MSc or equivalent)
  • Significant experience working in primary care
  • Confident managing minor illness autonomously
  • Strong clinical assessment and decision-making skills
  • Experience in managing long-term conditions in primary care
  • Experience working with QOF frameworks
  • Minor illness or urgent care background
  • Experience in using Systmone
