37 hours per week (Monday to Friday, 09.00 – 17.00)
Location:
Eaglestone Health Centre, Milton Keynes
- Support the NHS Mental Health SPA (Single Point of Access) Team to review and process referrals received from GP Practices and other sources, triage and assess them.
- Actively call patients to assist them through the process and secure onward appointments into community or secondary care service providers.
- Signpost patients and direct them to appropriate services, including secondary care, the local Voluntary and Community sector services, and activities that support patients’ mental health and wellbeing.
- Answer NHS 111 Mental Health calls, assess risks in real time, manage the call appropriately, and elevate when necessary.
- Provide administrative support to the Mental Health SPA for all services, respond to messages/voicemails, and liaise with clinical or team lead as appropriate.
- Monitor service inboxes during core office hours, responding to queries professionally with helpful information.
- Monitor and action tasks within Systm One, maintain accurate records of all activities, including outbound and inbound calls, and archive correspondence.
- Build and maintain working relationships with voluntary and community sector, relevant services and activities to keep signposting pathways current and develop new pathways.
- Deliver clinical and non‑clinical information in appropriate chronological templates to other services.
- Carry out health and safety responsibilities in line with Mind BLMK H&S policies, including risk assessments, reports, safeguarding, incidents/accidents, premises.
- Bring VCSE experience, knowledge and culture into an NHS team.
Minimum of 1 year of employment working in health, social care or information and advice, in direct contact with service users, ideally within a Mental Health setting, and experience working with challenging or vulnerable patients presenting with multiple or complex needs or risk history.
Experience with referral management and signposting, knowledge of NHS 111 Mental Health and ability to assess risks and manage calls is required.
