Essential Duties:

Under the general supervision of the Patient Care Department Manager, the Hospital Liaison – Care Coordinator plays a key role in cultivating strong relationships within multiple medical facilities. This individual is responsible for providing prompt service, educating patients and families on equipment use, coordinating medical documentation, and actively promoting and marketing Super Care Health’s full range of Durable Medical Equipment (DME), Respiratory therapies.

In addition to patient care coordination, this role includes sales‑related responsibilities such as identifying referral opportunities, strengthening relationships with healthcare providers, and increasing Super Care Health’s presence within assigned hospitals.

Responsibilities:
  • Serve as the primary point of contact between Super Care Health and hospital personnel, ensuring seamless coordination of DME and respiratory equipment deliveries to hospital rooms and patient homes.
  • Develop and maintain strong relationships with physicians, case managers, discharge planners, and other healthcare professionals to generate new business opportunities and drive referrals.
  • Promote and educate hospital staff on Super Care Health's DME, respiratory therapies, and infusion services to increase awareness and referrals.
  • Conduct regular facility visits to ensure visibility and accessibility for healthcare professionals and patients.
  • Provide timely consultation to patients and their families regarding equipment delivery, potential co‑pays, and usage instructions.
  • Request and ensure the accuracy of all necessary medical documentation for equipment orders.
  • Monitor and track referrals, ensuring Super Care Health is the preferred vendor for partnered healthcare systems.
  • Identify opportunities to expand service utilization and sales within assigned hospital accounts.
  • Conduct or participate in presentations, in‑services, and educational seminars for hospital staff and case managers to reinforce Super Care Health’s value proposition.
  • Maintain accurate records of interactions with hospital personnel, patient referrals, and sales activities, submitting required reports in a timely manner.
  • Ensure compliance with insurance pre‑authorization requirements and coordinate with Managed Care and Primary Medical groups as needed.
  • Maintain adequate inventory levels in assigned hospital storage areas to ensure prompt equipment availability as well as reconcile movement of equipment.
  • Collaborate with Customer Care Specialists and Dispatch teams to optimize equipment delivery and customer satisfaction.
  • Represent Super Care Health professionally at all times and adhere to all HIPAA, company, and industry‑related regulations.
  • Attend all department meetings and training sessions as scheduled.
  • Perform all other duties as assigned by department leadership.
Physical Demands:
  • Must be able to walk long distances across large healthcare facilities.
  • Must be able to lift and carry equipment and miscellaneous items up to 75 lbs. on a daily basis.
