We are seeking a compassionate Primary Care Provider to join our team, responsible for comprehensive patient care, including assessments, diagnoses, treatment plans, and collaboration with healthcare professionals.
Responsibilities:
- Provide comprehensive healthcare services and coordinate patient care.
- Assess, diagnose, and treat residents’ physical and mental well-being.
- Order diagnostic tests and prescribe medications.
- Collaborate with the interdisciplinary team for seamless care.
Requirements:
- Nurse Practitioner or Physician Assistant License.
- DEA License.
- 6 months experience in long-term care.
