.

The Lodge at Mills River is a patient-centered skilled nursing facility dedicated to providing exceptional long-term care, rehabilitation, and support services to our residents. We maintain a collaborative, caring environment that empowers our team members to deliver high-quality clinical care and advance professional excellence.

Key Responsibilities:

MDS Coordinator
  • Complete resident MDS assessments (admission, quarterly, significant change, and discharge) per CMS guidelines and state requirements.
  • Ensure accurate documentation that supports reimbursements and quality measures.
  • Interpret resident data to identify functional needs and care planning implications.
Interdisciplinary Coordination
  • Lead and coordinate the Resident Assessment Instrument (RAI) process with interdisciplinary team members (nursing, therapy, social services, dietitian).
  • Facilitate care conferences related to assessment findings.
  • Educate staff on documentation practices to support MDS coding.
Regulatory Compliance & Reporting
  • Maintain up-to-date knowledge of federal/state regulations and payer requirements (Medicare/Medicaid).
  • Prepare for and participate in state/federal surveys and audits as needed.
  • Monitor MDS submissions and adjust processes to maintain compliance.
Quality Improvement
  • Analyze facility quality indicators (QMs) and support performance improvement strategies.
  • Collaborate with clinical leadership to improve resident outcomes.
Qualifications & Skills

Required:

  • Current, unrestricted Registered Nurse (RN) license in North Carolina.
  • Experience with MDS 3.0 assessments in a SNF or long-term care setting.
  • Strong working knowledge of CMS RAI manual and quality measures.
  • Excellent clinical assessment and critical thinking skills.

Preferred:

  • Previous experience as MDS Coordinator or in Reimbursement/Utilization Review.
  • Experience with Electronic Health Records (EHR) and MDS software systems.
  • Certification in MDS (e.g., MDS 3.0 Certified Specialist).
