.
The Lodge at Mills River is a patient-centered skilled nursing facility dedicated to providing exceptional long-term care, rehabilitation, and support services to our residents. We maintain a collaborative, caring environment that empowers our team members to deliver high-quality clinical care and advance professional excellence.
Key Responsibilities:MDS Coordinator
- Complete resident MDS assessments (admission, quarterly, significant change, and discharge) per CMS guidelines and state requirements.
- Ensure accurate documentation that supports reimbursements and quality measures.
- Interpret resident data to identify functional needs and care planning implications.
- Lead and coordinate the Resident Assessment Instrument (RAI) process with interdisciplinary team members (nursing, therapy, social services, dietitian).
- Facilitate care conferences related to assessment findings.
- Educate staff on documentation practices to support MDS coding.
- Maintain up-to-date knowledge of federal/state regulations and payer requirements (Medicare/Medicaid).
- Prepare for and participate in state/federal surveys and audits as needed.
- Monitor MDS submissions and adjust processes to maintain compliance.
- Analyze facility quality indicators (QMs) and support performance improvement strategies.
- Collaborate with clinical leadership to improve resident outcomes.
Required:
- Current, unrestricted Registered Nurse (RN) license in North Carolina.
- Experience with MDS 3.0 assessments in a SNF or long-term care setting.
- Strong working knowledge of CMS RAI manual and quality measures.
- Excellent clinical assessment and critical thinking skills.
Preferred:
- Previous experience as MDS Coordinator or in Reimbursement/Utilization Review.
- Experience with Electronic Health Records (EHR) and MDS software systems.
- Certification in MDS (e.g., MDS 3.0 Certified Specialist).
