We are seeking a highly motivated and compassionate Registered Nurse (RN) to join our team at The Lodge at Mills River as a Full-Time MDS Coordinator.
The Lodge at Mills River is a patient-centered skilled nursing facility dedicated to providing exceptional long-term care, rehabilitation, and support services to our residents. The MDS (Minimum Data Set) Coordinator
, under the supervision of the Administrator, is responsible for the accurate and timely completion of the MDS assessments, coordination of the resident assessment process, and utilization of MDS data to support quality outcomes and regulatory compliance. The MDS Coordinator ensures all assessments reflect the resident’s condition and care plan in accordance with CMS and state guidelines.
- Complete resident MDS assessments (admission, quarterly, significant change, and discharge) per CMS guidelines and state requirements.
- Ensure accurate documentation that supports reimbursements and quality measures.
- Interpret resident data to identify functional needs and care planning implications.
- Lead and coordinate the Resident Assessment Instrument (RAI) process with interdisciplinary team members (nursing, therapy, social services, dietician).
- Facilitate care conferences related to assessment findings.
- Educate staff on documentation practices to support MDS coding.
- Maintain up-to-date knowledge of federal/state regulations and payer requirements (Medicare/Medicaid).
- Prepare for and participate in state/federal surveys and audits as needed.
- Monitor MDS submissions and adjust processes to maintain compliance.
- Analyze facility quality indicators (QMs) and support performance improvement strategies.
- Collaborate with clinical leadership to improve resident outcomes.
- Current, unrestricted Registered Nurse (RN) license in North Carolina.
- Experience with MDS 3.0 assessments in a SNF or long-term care setting.
- Strong working knowledge of CMS RAI manual and quality measures.
- Excellent clinical assessment and critical thinking skills.
- Previous experience as MDS Coordinator or in Reimbursement/Utilization Review.
- Experience with Electronic Health Records (EHR) and MDS software systems.
- Certification in MDS (e.g., MDS 3.0 Certified Specialist).
