Nurse Practitioner (NP) Job Location

STGi is currently seeking a Nurse Practitioner to provide services at the Georgia War Veterans Home. The primary purpose of this position is to provide primary care to specific residents under the medical direction and supervision of the residents' attending physicians or the Medical Director of the facility with an emphasis on assessment, illness prevention and health care management including assistance in modifying the treatment regimen to meet the physical and psychosocial needs of the resident in accordance with established medical practices, requirements of the state and the policies and goals of the facility.

• Contribute to the facility's quality management including reduction of re-hospitalizations that were avoidable, development of safe discharge plans and culturally competent care.
• Assist the Quality Assurance and Performance Improvement (QAPI) Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
• Assist in the development and implementation of facility policies and procedures as appropriate.
• Initiate requests for consultation or referral.
• Participate in facility surveys (inspections) made by authorized government agencies as necessary.
• Participate in the development of the annual facility assessment by assisting in identifying tools, training and resources required to effectively treat the resident population.

• Assess, diagnose and treat residents with acute, episodic or chronic illness independently or as part of a healthcare team.
• Order, interpret and evaluate diagnostic tests to identify and assess resident's clinical problems and health care needs.
• Consult with the Medical Director and the attending physician regarding specific residents as necessary.
• Determine when to refer the resident to a physician or specialist for evaluation, supervision or directions.
• Collaborate with the interdisciplinary team (IDT), resident and family members on care planning and interventions that are individualized and person-centered; evaluate interventions and revise as appropriate.

• Coordinate with physicians and the Medical Director for on-call rotation to provide appropriate

• Must possess, as a minimum, a Master of Science in Nursing (MSN) from an accredited college or
