Health Care Case Manager (RN), Essex South-Millburn, NJ
Health Care Case Manager (HCCM) for the Child Health Program within the School of Nursing at Rutgers Biomedical and Health Sciences. Supports the Department of Children and Families in meeting health care needs of children in out‑of‑home placement at the Essex South office.
Key Responsibilities:
- Assist with enrollment of a child in a managed care entity and identify a medical home upon entry to out‑of‑home placement.
- Assess a child’s health care status and needs by retrieving and reviewing all medical records and interviewing past providers and caregivers; forward information to the medical home and other health providers.
- Schedule an initial physical health evaluation with caregiver and DCF/CP&P, review preliminary and final reports, and ensure recommended health care is completed.
- Develop a child health care plan in collaboration with the medical home, specialists, caregivers, community service providers, and when appropriate, DCF/CP&P.
- Link the child and caregiver with appropriate health resources and community services.
- Ensure the child attends timely EPSDT visits and specialty appointments; review reports and forward necessary information to the medical home.
Qualifications:
- Current licensure as a Registered Nurse in the State of New Jersey.
- Bachelor’s Degree in Nursing or a BA/BS in a health‑related field; may substitute years of pediatric nursing experience for degree.
- Minimum 1‑2 years of full‑time clinical experience as a pediatric nurse in pediatric environments (pediatric doctor’s office, hospital unit, public health nursing, emergency department, critical care unit, family practice, school nurse, child welfare nursing, juvenile mental/behavioral health facilities, or children’s hospital).
- Current certification in American Heart Association Basic Life Support for Healthcare Providers.
- Vehicle and valid New Jersey license required for off‑site visits.
Preferred Qualifications:
- Two (2) years of experience in nursing case management, community health nursing, or nursing care of vulnerable families.
Physical Demands and Work Environment:
- Standing, bending, and walking (up and down stairs) and ability to lift 50 pounds (infants/children, records) to waist level.

Compensation: Salary ranges from $71,162 to $101,456 with $85,428 as the mid‑range. The final offer may vary based on qualifications, experience, and grant funding.
Position Status: Full Time – 37.50 hours per week, Day shift.
