Job Description Summary
The Clinical Pharmacist – PT, PD, CA is a licensed professional who provides safe and appropriate pharmacy services to Option Care patients in accordance with policy, professional standards and applicable regulations. This entry level role in clinical pharmacy in the alternate setting coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff and other professionals on medication use.

Job Responsibilities
  • Determines the suitability of individual patients for home care. Collects and organizes patient-specific information to determine suitability for home care in accordance with Option Care policies; assesses medical condition and prescribed therapy for home care with defined outcome goals.
  • Designs, recommends, monitors and evaluates patient-specific pharmacy care plans for home care patients. Recommends modifications to therapy or monitoring parameters as needed and identifies appropriate monitoring parameters. Completes nutritional assessments and provides recommendations.
  • Modifies pharmacy care plans based on evaluation of monitoring data and patient information. Applies pharmacokinetic principles and assesses patient laboratory values.
  • Provides medication-use education to Option Care staff, professionals, patients and caregivers. Uses effective patient education techniques for counseling.
  • Ensures continuity of pharmaceutical care to and from the home and other patient-care settings. Communicates pertinent information, prepares and dispenses medications, calibrates equipment, ensures proper labeling and storage, and adheres to safety and quality practices. Ensures compliance with state/federal laws, USP standards and DEA regulations, including controlled substances obligations.
  • Manages medication administration equipment and related devices. Programs and advises on equipment use, participates in incident management and reporting, and maintains inventory accuracy.
  • Responds to drug information requests from health care providers and patients. Analyzes literature to formulate responses and pursues continuing education requirements.
  • Manages investigational drug products per regulations and Option Care policies. Prioritizes workload and ensures accuracy of work by pharmacy support personnel.
  • Complies with accreditation, legal, regulatory and safety requirements; maintains patient confidentiality and ethical guidelines.
  • Performs other duties as assigned as needed.
Supervisory Responsibilities

Education and Experience
  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license as granted by the State Board of Pharmacy.
  • One of the following: (a) at least one year of sterile IV admixture experience, (b) knowledge of non-sterile compounding, immunizations and MTM, (c) completion of an ASHP accredited Pharmacy Residency, (d) completion of an Advanced Pharmacy Practice Experience, or (e) equivalent experience as approved by leadership.
Qualifications
  • Reliable transportation to make home visits; valid driver’s license and proof of insurance if using a personal vehicle.
  • Experience delivering customer service to internal and external customers and meeting quality standards.
  • Basic Microsoft Word and Excel skills; basic PowerPoint skills for creating and presenting information.
Travel
  • Willing to travel up to 5% of the time for business purposes (within state).
Preferred Qualifications
  • Completion of an ASHP-accredited Pharmacy Residency or APPE with an accredited school of pharmacy.
  • Experience in home care, hospital or clean room settings.
Management
  • At least 1 year of indirect management experience, including staff development, training and assignment of work.

Due to state pay transparency laws, the full salary range is provided below: salary to be determined based on education, experience, knowledge, skills and abilities, and internal equity. Pay Range: $46.70-$77.83
