Milford, MA. Job Type: Per Diem Independent Contractor, 1099 Contract. Pay Rate: $130/hr for onsite patient care, $100/hr for virtual lab review.

Schedule:

Tuesday 5/12/26 2pm to 11:30pm, Wednesday 5/13/26 6am-4pm, Thursday 5/14/26 4am-1:30pm.

As an Administrative Agent for the Program, Vital Check Wellness is seeking a Nurse Practitioner (NP) to provide general wellness checkups on site at a private business in Milford, MA. Our office arranges the patient appointments and handles administrative work, giving you the time to provide clinical one‑on‑one patient care (usually no more than 20 patients per day). Vital Check does not provide medical care and does not control clinical decision‑making.

• Provide general wellness checkups.
• Consult with patients to understand their health concerns.
• Advise or refer patients for follow‑up care.
• Perform blood draws for patients.
• Provide telemedicine services for lab review follow‑ups.

• Active clean Nurse Practitioner license to practice in Massachusetts.
• Family Nurse Practitioner (FNP), FNP‑C, FNP‑BC, or Acute Care Nurse Practitioner (ACNP) certified.
• Active BLS/ACLS certification required.
• A collaborating physician is required unless state requirements for independent practice are met.
• Background in internal medicine, family practice, urgent care, or emergency room preferred.
• Must be able to perform phlebotomy.
• Proficient with computers, customer service‑oriented, and able to work independently.
• Focused on preventive medicine and patient education.
• Personal laptop or tablet required.
• Must be able to reliably commute to Milford, MA.
