The Registered Nurse Case Manager is responsible for admitting patients for home care services, developing the patient's plan of care, coordinating with the patient's provider and other disciplines for the provision of services, and ensuring the safe delivery of patient care.

Responsibilities- Assess/reassess patients and establish plans of care that are patient specific.
- Collaborate with providers, other disciplines, and communicate all changes promptly.
- Assess payment sources and ensure ongoing authorizations as necessary.
- Educate and instruct patients/caregivers in their ongoing medical maintenance. Begin discharge planning on admission.
- Complete all documentation within 24 hours. Must include changes in condition, follow-up with provider or other disciplines with supporting documentation.
- Evaluation of patient progress and prompt action.
- Oversight of LPNs and Home Health Aides.
- Schedule patients independently according to the individual plan of care.
- Attend all staff meetings, in services, and other required meetings. Compliance with all mandatory requirements.
- Follow policies and procedures.
- Thorough and accurate OASIS documentation.
- Coordinate and manage the care of patients receiving services in their home.
- Participate in case management conferences and document per guidelines.
- May perform other duties as assigned.
EDUCATION
Nursing Diploma or Associates Degree in Nursing. Bachelors in Nursing preferred.
EXPERIENCE
Minimum two (2) years of nursing experience in an acute care setting. Previous home care experience is desirable but not required.
LICENSURE
Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.
SPECIAL SKILLS
Excellent clinical knowledge, verbal/written skills, and organizational skills. Must be able to work independently. Able to competently assess patients' needs and follow through accordingly. Strong ability to work with providers and members of the multidisciplinary team.
PHYSICAL DEMAND
Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.
