Position Description
Provides individual, family, and group counseling services to patients to address various mental health and wellness-related issues. Conducts assessments and helps patients to problem‑solve areas of need. Helps to develop coping strategies and action‑based goals and objectives for patients. May specialize in a particular area of counseling.
Major / Essential Functions- Provide brief, evidence‑based therapy to patients referred through collaborative care pathways.
- Supervise mental health fellows weekly; provide feedback and assess progress using structured tools.
- Coordinate with PCPs, case managers, and other clinical staff to ensure continuity of care.
- Participate in team meetings, program development, and clinical documentation audits.
- Assist in the training and onboarding of new fellows, including policy and documentation orientation.
- Promote trauma‑informed, culturally competent care across service sites.
- Education:
Master's degree in psychology, counseling, social work, or related field. - 3 years of related experience.
- Licenses / Certificates: LPC, LPC‑A, LCSW, LMSW (must be working towards LCSW), LMFT, LMFT‑A, LPA.
- Experience with CoCM or integrated care strongly preferred.
