• Provide care in nursing and long-term care facilities
• Manage chronic conditions, perform full assessments
• Close care gaps (HEDIS and STAR measures)
• Lead care planning with facility teams
• Support and educate patients’ families

• Active, unrestricted APN license in Michigan
• Board certified and Medicare credentialed
• 6+ months in skilled nursing or long-term care
• Solid with digital tools, team-based care
• Able to work onsite in Midland, MI

• Pay: $123K–$135K base + $15K bonus
• Insurance: 90% medical/dental/vision (70% for dependents)
• Other: $10K life insurance, HSA/FSA, 401(k) match
• Time off: Up to 160 hours PTO + 10 holidays
• Growth: Paid CE days, stipend, full learning tools
