Summary: The Substance Use Navigator is responsible for providing coordination and support to Premier Health Emergency Department / OB Triage patients and patients admitted to the medical floors who have been identified with substance use disorder (SUD). The Navigator supports linkage and warm handoff for Medications for Addiction Treatment (MAT) from the ED and inpatient medical floors. This includes regular SUD assessments, linking to ongoing treatment and community resources, educating patients, communicating with clinical and administrative staff, monitoring and reporting outcomes, participating in grant-related meetings and activities, and ensuring follow-up with the identified patient population.
Status: Full-Time / 80 hours per pay period
Responsibilities- Provide coordination and support to SUD-identified patients in the ED, OB Triage, and inpatient medical floors.
- Perform regular SUD assessments and establish linkages to ongoing treatment and community resources.
- Facilitate warm handoffs for MAT services from the ED and inpatient settings.
- Educate patients on treatment options, resources, and next steps.
- Communicate effectively with clinical and administrative staff; monitor and report outcomes.
- Participate in grant-related meetings and activities as required.
- Ensure regular follow-up with identified patient populations.
Education

Qualifications:
Associate Degree in Chemical Dependency, Allied Health, Social Services, or Social Work - Position-Specific Testing Requirement: N/A
Licensure/Certification/Registration
- Chemical Dependency Counselor Assistant (CDCA) or obtain within first 6 months of employment
Experience
- Minimum

Experience:
Community Health Worker, Peer Recovery Supporter, Case Manager, or Emergency Department experience - Preferred Experience:
- Medical terminology
