**** JOB SUMMARY
** The CKP Program Specialist will work closely with the CKP Project Director for at least half of the position and is responsible for the project coordination of multiple CKP education interventions. Education interventions for CKP may include ECHO, learning collaboratives, webinars, technical assistance, consultation, and other innovative interventions. The CKP Program Specialist will also work closely with the CBS Operations Manager to provide various modes of support to programs within CKP including but not limited to the Ryan White Program, Healthcare for the Homeless Services, and the Mobile Health Program.

The CKP Program Specialist is a key member of various community-based and clinical teams within CKP and is responsible for supporting the development, design, implementation and evaluation of educational interventions for primary care providers, care team staff, and clinical, operational, and executive leaders in primary care and other health care organizations. Specific responsibilities include project coordination for a variety of educational interventions in various stages of planning and implementation;

work with CKP staff to develop planning and outreach materials; engagement with participants involved in the various interventions to build and maintain learning communities; scheduling for all planning meetings between CKP/CHC staff and external organizations; manage online learning network for participants and staff; tracking and monitoring of operational data including continuing education, survey administration, data collection; data analysis and reports for continual improvement of education interventions;

participation in proposal development, grant writing, poster presentations, and publications. Delivery methods for educational interventions include video conferencing, video recordings, site visits, and in-person/virtual consultations.
** ROLE AND RESPONSIBILITIES
*** Responsible for efficient management of schedule for all sessions including the arrangements and maintenance for various education interventions.
* Providing clerical, organizational, and technical support for CKP leadership, session participants, and faculty.
* Developing engaging and attractive presentations for educational sessions, outreach efforts and conferences.
* Supporting coordination and project organization on cross-CKP/CHC initiatives, as requested or needed.
* Assisting the project Director in the negotiation and development of new programs and contracts, including coordination of completed execution.
* Organizing and communicating with external organizations to determine appropriate educational intervention methods.
* Facilitating regularly scheduled planning meetings with CKP staff/leaders.
* Scheduling and effectively running regular meetings with external organizations as needed.
* Supporting senior staff with grant development and reporting, which may include writing parts of the grant applications or reports, brainstorming elements of a work plan, coordinating with partner organizations for letters of support, and proofreading for grammar and format.
* Working with the senior staff to manage grant deliverable timelines.
* Creating operational reports for external funders and supporting with other deliverables.
* Assist with creating content for marketing purposes in collaboration with CHC’s Communications team including eBlasts, newsletters, printed collateral, and maintaining/updating assigned CKP webpages.
* Supporting the recruitment of participants (students, residents, providers, care team staff, leaders) and organizations and tracking admissions/enrollment/graduation.
* Recording and maintaining education intervention data: attendance, didactics, case presentations, didactic schedule via web-based program, evaluation surveys.
* Administering surveys and assisting with data collection and analysis to maintain quality of education programs.
* Actively participate in all CKP/CHC meetings and provide up to date information on projects while also contributing ideas for innovative advances and enhancements to program as often as possible.
* Work with the CKP evaluation team to design assessment tools and evaluation materials in an effort to ensure critical deadlines are met and gaps are identified as quickly as possible.
* Maintain quality improvement training, as assigned, to effectively work with CKP/CHC and external teams in a manner that facilitates a culture of Quality Improvement.
* Assisting with onboarding of new learners to educational programs including conducting technology tests and providing technical support as needed.
