Position:

About Helen Porter Rehabilitation & Nursing:
Helen Porter Rehabilitation & Nursing is a welcoming, secure residence where residents receive skilled nursing care, daily living assistance, and respect for privacy and individuality. We are a 98-bed, state-licensed facility in Middlebury, VT, certified by Medicare and Medicaid, specializing in a home-like environment. The facility has been renovated and features a courtyard, water fountain, gardening, pergolas, and circadian lighting.
We have three units and offer part-time or per diem opportunities across these units through this application.
Unit and Program OverviewLong-Term Care Unit — provides nursing care and daily living assistance with activities tailored to resident needs while maintaining a long-standing tradition of quality care.
Memory Care Unit — designed for residents with dementia, offering activity-based care in a warm, supportive, and safe environment. This unit serves residents who are most vulnerable and may require advocates and dedicated team support.
Post Acute Care Unit — focuses on goal-directed care after surgery or hospitalization, with programs designed to help patients return home as quickly as possible and regain independence. Conditions commonly treated include strokes, neurological and orthopedic conditions, respiratory issues, cardiac and post-trauma situations.
Therapy services are provided Monday through Friday, with potential six-day coverage if needed to maximize outcomes. Nursing staff support therapies when therapy staff are not present.
Requirements- Current RN licensure recognized by the State of Vermont required.
- Appropriate experience in a specific clinical area (varies by unit).
- Sign-on bonus up to $10,000 (pro-rated for part-time)
