Clinical Nurse Manager

The Clinical Manager at Porter Medical Group will collaborate closely with the UVMHN Porter Medical Group Practice Leadership Team and Physician Leaders to ensure delivery of consistent, high-quality, and affordable health care. This role promotes timely access to care, supporting patients and communities in achieving their health goals.

The Clinical Manager will provide leadership of clinical operations across Porter Medical Group, including oversight of clinical workflows, policies, and ambulatory resource pool. Responsibilities include developing and maintaining clinical policies and procedures, optimizing clinical workflows, ensuring regulatory compliance, leading outpatient clinical orientation and competency development, performance management, and staff development. This role fosters a culture of empowerment and excellence that enhances the patient experience.

• Driving collaboration between Primary Care and Specialty Care teams.
• Utilizing data to inform clinical decision-making and proactively manage patients at risk or with rising risk.
• Providing expert guidance on ambulatory clinical workflows and best practices to support population health and chronic disease management.
• Supporting CAP Program and CEU activities for Nurses and clinical staff.
• Working with EPIC, Nursing, and Clinical Ops teams; partners with Primary and Specialty Care.
• Partnering with UVMHN Medical Group Operations Leadership on clinical and nursing practice policy and procedures.
• Partnering with MG Network Director Clinical Operations and MG VP Clinical Operations on broad ambulatory nursing initiatives.

• Bachelor’s Degree in Nursing. Active Vermont RN, BSN (required).

• Must have a minimum of 2-3 years of nursing experience, preferably in ambulatory/outpatient setting, with progressive leadership experience in a health care management setting.
