Days 8a-5p M-F

Must have EMR: HCHB (Home Care Home Base) experience

Must have previous/recent Hospice experience

ASAP start, 13 week assignment

Start ASAP for 13 weeks M-F, days Coverage area:
Westport to Old Saybrook May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address Hospice experience required Productivity expectation is 24-30 visits/wk for 40 hours. Home Care Home Base experience required Client may consider without HCHB Any overtime must be pre-approved and requires written approval from the Administrator
