, Trach Care
, TF/PEG Care
, Catheter Care

. Our 24/7 support model ensures you re never alone in the field and are backed by a responsive, reliable office team.

The schedule for this position is 7:00 AM – 7:00 PM, Any day between Monday through Sunday
, offering a full-day, consistent schedule while making a meaningful difference in a client s life.

• Provide Adult Care for clients, including assistance with daily living activities such as bathing, dressing, grooming, and mobility.
• Perform Trach Care safely and accurately, monitoring for complications.
• Manage TF/PEG feedings
  , including preparation, administration, and documentation.
• Provide Catheter Care
  , ensuring cleanliness, proper function, and accurate records.
• Deliver Wound Care
  , including dressing changes, cleaning, and observation for signs of infection.
• Monitor clients  overall health and report any changes promptly to the supervising nurse or healthcare team.
• Maintain accurate documentation of all care provided.
• Follow individualized care plans to ensure clients  
  comfort, safety, and dignity
  .
• Communicate effectively with clients, families, and the healthcare team.
• Adhere to all infection control, safety, and hygiene protocols.
• Work consistent full-day shifts from 7:00 AM - 7:00 PM, Any day between Monday through Sunday

• Perform all skills, treatments, and procedures to the scope defined in the Nurse Practice Act – Ohio Revised Code (ORC) of clinical practice
• Implement treatment and plan of care with sound judgment and critical thinking
• Maintain accurate, detailed, and timely documentation consistent with the plan of care
• Document observed changes in patient s condition and communicate to the Director of Nursing
• Adhere to agency policies, procedures, and protocols
• Adhere to HIPAA laws and ensure confidentiality and security of client information

• Unencumbered license in the State of Ohio
• Minimum three (1) year of documented clinical experience; private duty, home care, assisted living, or skilled nursing experience preferred
• Strong clinical skills; self-directed with ability to work with little supervision
• Certified CPR/BLS and negative TB skin test or chest x-ray
• Licensed driver with reliable and insured automobile (proof of insurance required)
• Ability to read, write, speak, and understand English and communicate effectively
• Ability to stand, walk, squat, bend, kneel, reach, twist, push and pull; varying mobility support as needed
• Must be able to lift up to 50 pounds
• Up-to-date vaccinations including COVID-19, Influenza, and Hepatitis B
