The Hospice Social Worker is responsible for providing initial psychosocial assessments, plan of care, ongoing psychosocial counseling, bereavement services, community education and outreach and referral to Hospice patients and families.
MINIMUM QUALIFICATIONSEducation:
Master's Degree in Social Work and is a Licensed Independent Social Worker.
Required length and type of experience:Minimum of three (3) years of clinical work experience preferably in a healthcare setting or other related field.
