RN - Home Health

• Work 8-hour day shifts roughly 8:00 AM to 4:30 PM with flexibility depending on census
• 36 hours flexible to 40 hours per week
• 13-week contract starting ASAP
• Weekend call rotation and holiday call rotation required
• Weekday call 1-2 nights as needed
• Travel requirements include 20-25 patients weekly with 4-5 visits per day on average
• Daily mileage can vary from 25-60 miles depending on territory, with potential for over 100 miles due to large territory coverage
• Mileage reimbursement provided at current government rate
• Must live outside 50 miles from the facility (radius rule)
• Required certifications: AHA BLS, Active MI and WI RN license, Valid Driver's license
• OASIS certification is a plus
• Minimum 2+ years home health experience and travel experience required
• OASIS and Epic charting experience required
• Perform OASIS admissions, all time points, episode management, doctor calls and coordination of care
• Clinical competencies required in wound care, IV therapy, ports, lab draws, and chronic disease management
• Timely documentation in EMR system required
• Associate degree or certificate required, Bachelor's degree preferred
• Contract eligible for 36 hours of unbillable low census hours
• May be placed on call for low census hours
