As a Care Manager RN, you’ll play a key role in coordinating care for patients with complex medical and social needs. Your work will help patients navigate the healthcare system and achieve better long-term outcomes.
Key responsibilities include:
- Coordinating and monitoring patient care to optimize health outcomes
- Identifying and supporting high‑risk patients
- Developing and managing individualized, patient‑centered care plans
- Providing patient education and motivational interviewing to promote self‑management
- Collaborating with primary care providers, specialists, hospitals, and community resources
- Conducting post‑hospital follow‑ups, medication reconciliation, and care transitions
- Managing a caseload (up to 250 patients) with a focus on prevention and continuity of care
- Maintaining accurate documentation and supporting care management billing
- Clinic/office‑based setting
- Occasional travel between clinic locations
- Physical requirements include standing, walking, and lifting up to 25 lbs
- Non‑exempt position; occasional hours over 40/week may be required

🎓 What We’re Looking For
- CPR/BLS certification
- Bachelor’s degree in Nursing (or equivalent experience)
- Strong communication and organizational skills
- Experience or interest in care coordination, population health, or chronic disease management
- Familiarity with FQHCs and/or the Patient‑Centered Medical Home (PCMH) model is a plus
- Comfort using EHRs and standard office software
