Certified Nurse Practitioner (NP) – Post-Acute Care | Salary up to $140K

Base pay range: $115,000 - $140,000 per year + uncapped bonus potential.
Job type: Full-time.

Altea Healthcare is seeking a Certified NP for post-acute and skilled nursing care. We provide training, mentorship, and support for both experienced providers and new graduates.

As a key clinical provider in a skilled nursing facility, you will diagnose, treat, and guide patients to better health. No two days are the same.

• Perform physical exams and review medical histories.
• Order and interpret diagnostic tests (labs, imaging, etc.).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.

We welcome both experienced providers and motivated new graduates.
NP License (or eligibility to obtain) is required. All experience levels are welcome; training and mentorship are available. Strong passion for geriatrics, internal medicine, or primary care; excellent communication skills; patient‑first approach.
