Optalis Health & Rehabilitation of Three Rivers provides a comprehensive range of rehabilitation services, including physical, occupational, and speech therapy, along with long‑term and respite care options. We are seeking a motivated and compassionate Registered Nurse (RN) to join our dedicated healthcare team. The RN plays a key role in providing quality nursing care, supervising clinical staff, and ensuring the physical and emotional well‑being of each resident in accordance with facility policies and state and federal guidelines.

• Provide licensed nursing care to assigned residents as ordered by the physician and in accordance with facility, federal, state, and local regulations.
• Conduct and document resident assessments, observations, and interventions.
• Communicate effectively with residents, families, physicians, guardians, governmental agencies, and facility staff as required.
• Ensure accurate and timely documentation of resident status and care provided.
• Maintain compliance with infection control policies, safety standards, and facility procedures.
• Assist in training and mentoring other nursing staff to promote a culture of teamwork and excellence.
• Demonstrate professional behavior, empathy, and respect in all interactions with residents and team members.

• Current Registered Nurse (RN) License in the State of Michigan.
• Current CPR and BLS certifications required.
• Strong communication and documentation skills.
• Ability to multitask effectively and remain calm under pressure.
• Prior experience with Point Click Care  preferred.
• Knowledge of infection control standards, policies, and procedures.
• Previous long‑term care or skilled nursing experience preferred.
