Optimal Care Nurse Practitioners provide primary and/or consultative professional care in a variety of care settings, focusing on geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
- Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
- Determine follow‑up assessments according to patient needs and best practice guidelines.
- Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care.
- Assess health status by obtaining relevant health and medical history.
- Perform a physical exam based on age and patient history per the Standard Care Arrangement with a collaborating physician.
- Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient's clinical problems and healthcare needs.
- Perform diagnostic or therapeutic interventions consistent with certification, law, and state medical privileges.
- Prescribe and monitor medications as allowed by prescriptive authority through the appropriate state board agency.
- Develop a treatment plan in collaboration with the patient and caregivers.
- Refer patients to physicians as appropriate for consultation or for specialized health resources and treatment.
- Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas.
- Provide patients and families an understanding of diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits.
- Hold case conferences with families when appropriate to advance the care plan.
- Meet minimum productivity requirements based on practice standards.
- Master's or doctoral degree as a Board‑Certified Nurse Practitioner.
- Current state license as a Nurse Practitioner.
- Nursing skills as defined by accepted nursing standards.
- Ability to sit, stand, bend, lift, and move intermittently.
- Must be able to lift at least 35 lbs and bear the weight of an average adult effectively.
- Current and valid driver's license.
- Reliable transportation and valid automobile insurance coverage.
