The Care Manager provides services to individuals with severe and persistent mental illness, serious emotional disturbance, and/or intellectual or developmental disabilities. This role ensures individuals receive the necessary supports to achieve community inclusion, independence, and meaningful participation in home and community-based settings. Responsibilities include advocacy, service coordination, linkage to supports, and ongoing monitoring to ensure quality, continuity, and accessibility of services.
Core functions include service planning, assessment, coordination, and documentation.
- Develop, coordinate, monitor, and document person-centered service plans.
- Conduct comprehensive annual assessments identifying strengths, needs, and supports across all life domains.
- Coordinate with treatment teams, natural supports, and other disciplines to ensure appropriate evaluations and services.
- Lead the annual person-centered planning process and provide ongoing evaluation of service plans.
- Link individuals to internal and external services based on assessed needs.
- Advocate for individuals and collaborate with families, community partners, and service providers.
- Monitor service provision to ensure fidelity to the person-centered plan and modify plans as needed.
- Review provider reports and compile quarterly summaries reflecting progress toward goals.
- Provide preventive and supportive interventions to promote empowerment and self-determination.
- Serve as a liaison with courts, hospitals, and other institutions to support continuity of care.
- Maintain accurate and compliant documentation, data, and case records in accordance with Medicaid, State, Regional, and agency standards.
- Manage waiver certification and re-certification processes.
- Provide consultation and assistance to Direct Support Staff.
- Participate in committees, community engagement activities, and Emergency Services On‑Call (as applicable).
- Travel within the service area and statewide as needed; work flexible hours, including evenings, weekends, or holidays when necessary.
- Work collaboratively with staff, individuals served, and external partners.
- Maintain high standards of professionalism, productivity, and competence.
- Support orientation and training efforts for new staff.
- Perform additional duties as assigned.
Education & Experience
- Bachelor’s degree in a human services field recognized by the State of Michigan for Care Management.
- Minimum of one year of experience working with individuals with mental health conditions.
Knowledge, Skills & Abilities
- Proficiency with computers, Microsoft Office, and standard office equipment.
- Strong written and verbal communication skills with the ability to document accurately.
- Sound judgment, critical thinking, and analytical skills.
- Excellent organization and time‑management abilities.
- Knowledge of Medicaid, Waiver, and Level of Care guidelines.
- Ability to support the implementation of new requirements and program developments.
Licensure & Certifications
- Certification as a QMHP, QIDP, CMHP, or applicable Michigan licensure.
- Maintain all required trainings and certifications.
- Valid U.S. driver’s license and vehicle insurance per Michigan law.
- Proof of U.S. citizenship or authorization to work in the United States.
