Schedule: 40 hours per week | Mon – Fri | 8am‑4:30pm
Department: PACE of Southwest Michigan
In this role, you will support therapeutic and recreational activities both at our day center and through home visits, as directed. You’ll assist in planning and leading fun, meaningful activities while working alongside a supportive and dedicated interdisciplinary team. Every day offers the opportunity to make a real difference in the lives of those we serve.
Primary Responsibilities- Assist in the implementation of therapeutic activity programs to include group or individual recreational and social activities.
- Provide input into program plans.
- Assist in developing a monthly calendar of events for participants.
- Maintain general order in the activities area, including daily clean‑up, routine supply inventory, and ordering for the recreation department.
- Plan and implement activities for dementia‑specific participants.
- Build a sense of community and a warm social atmosphere in the Day Center in which participants can identify a support system.
- Complete progress reports and assessments.
- Communicate with facilities to assess participant activity and involvement.
- Attend IDT morning meeting, care, team huddles, and care conferences as needed.
- Assist with transporting and escorting participants to and from activities in the Day Center.
- Complete SBAR form for any clinical participant concerns when in the Day Center. >
- Assist with lunch (passing trays, serving drinks) as needed.
- Document in behavior logs as needed.
- Complete home visits when directed.
Provide reception desk coverage as needed.
- Minimum of one year of experience working with frail and elderly individuals.
- CPR certification or willingness to obtain upon hire.
- Valid, unrestricted driver's license.
