ShiftsRotating 8-hour Shifts (days, evenings, occasional nights). Rotating weekends required.
Responsibilities- Participates in interdisciplinary patient care rounds, collaborating with doctors, nurses, and other healthcare professionals to help maximize patient outcomes.
- Compares patients' medication history obtained against inpatient orders to ensure accuracy.
- Ensures medication regimens are appropriate and follows hospital‑established policies and evidence‑based guidelines. This includes, but is not limited to, antimicrobial use guidelines, renal dosing, and serum drug concentration protocols.
- Reviews all medication orders for appropriate dose, duration, frequency, indication, drug interactions, and contraindications.
- Works with medical providers to resolve discrepancies and documents outcomes.
- Serves as a medication expert, providing drug information to clinical staff.
- Compounds sterile intravenous preparations while adhering to USP 797 and USP 800 regulations.
- Ensures compliance with state regulations, federal regulations, Joint Commission medication management standards, CMS conditions of participation, and mentors technician staff to ensure safe and accurate medication handling and dispensing.
- Doctor of Pharmacy (Pharm
D) degree from an accredited pharmacy school. - Valid state license to practice as a pharmacist in New York.
- 1 to 2 years of hospital experience preferred, not required.
