URGENT:
Home Health Nurse – LPN/RN – Centria Autism

We are seeking a compassionate, highly skilled nurse to provide 1‑on‑1 care for a mature female patient with complex medical needs in a warm, family‑oriented home environment. The patient is a beloved matriarch of a large, supportive family and requires a dedicated nurse to manage multi‑system health challenges.

• 8:00 am – 8:00 pm (PRN)
• 8:00 pm – 8:00 am:
  Wednesdays, Thursdays, every other Friday & Saturday

• 24/7 private duty nursing for a mature female client in a beautiful, smoke‑free, pet‑free home.
• 12‑hour shifts (tentatively 8 a.m. – 8 p.m. or 8 p.m. – 8 a.m.)
• Days and nights available.
• Flexible scheduling with no requirement to work a certain amount of weekends or holidays.

• Tracheostomy care
• Total care (non‑weight‑bearing)

• Oxygen PRN
• Type II diabetes with insulin
• Bilateral foot drop, diabetic neuropathy, chronic UTIs
• History of stroke and TIAs

• Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.).
• Monitor and document vitals, symptoms, and changes in condition.
• Assist with ADLs such as repositioning, hygiene, mobility, etc.
• Support patient well‑being with compassionate and attentive care.
• Collaborate with families, caregivers, and the healthcare team.

• Active RN or LPN license in the state of Michigan.
• Experience in home health, private duty, or long‑term care preferred.
• Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required).
• Strong communication skills and a passion for patient‑centered care.

• Full‑time, part‑time, and per diem shifts available (Day, Night, and Weekend shifts).

We have immediate needs for private duty nurses (RNs or LPNs) in the West Bloomfield Township area to provide home care to pediatric patients. Our pediatric population ranges in ages from birth to 21 years of age.
