WHAT YOU CAN EXPECT
The PACE Clinic Nurse provides direct and indirect nursing care to assigned patients in clinic or community setting. This role works under the supervision of the Clinic Manager but works independently in the application of advanced nursing knowledge and skills. The nurse manages complex clinical situations for which he/she is responsible.

* Provides skilled nursing care and prescribed treatments in participant homes, at the PACE Center, and at all places where participants reside, including in assisted living and in skilled nursing facilities.
* Conducts nursing assessments and care planning according to regulatory requirements such as: initial assessments, change in condition assessments, semiannual assessments.
* Conducts intake assessments, including collecting all necessary consents, demographic information, compiling all outside medical records, building medical chart within EPIC, and documenting all collected information.
* Provides routine nursing assessment and care for chronic or acute illnesses according to the plan developed by the interdisciplinary team.
* Serves in an on-call rotation to troubleshoot, advise, teach and coordinate participant care needs outside of clinic hours.
* Manages medications within the center and clinic by filling pill boxes, administering medications and accurately completing documentation process.
* As a member of the Interdisciplinary Team, collaborates with team members to ensure that medical needs of each participant are met.
* Reports changes in the participant's condition to appropriate members of the Interdisciplinary team.
* Participates in care plan meetings, Interdisciplinary team meetings, and referral meetings.
* Coordinates participant care with outside health care personnel in order to maintain plan of care.
* Communicates effectively with hospital departments to minimize hospital lengths of stay and to coordinate a smooth discharge into the post-acute level of care.
* Case management: communicates with personnel from outside facilities, participates in discharge planning, manages participant recertification.
* Establishes and maintains cooperative working relationships with other program coworkers, contract agencies and outside organizations.
* Obtains and manages outside medical records.
* Demonstrates clinical skills regarding application of Franciscan Senior Health and Wellness PACE policy and procedures, physical assessments, symptom management and participant relationships.
* Maintains compliance with nursing competencies.
* Demonstrates a working knowledge of nursing contribution to the PACE quality program.
